Before Your First Session
To get started, please complete the two required forms below before your first session. These forms help establish expectations, clarify goals, and ensure we begin the coaching process with purpose and understanding.
Client Intake Form
This form provides background on where you are, what you want to work on, and what you hope to gain from coaching. It helps me prepare to support you in a focused and meaningful way.
Coaching Agreement
This agreement outlines the coaching relationship, expectations, confidentiality, communication, payment, and policies. Please review and complete it before your first session.
Confidentiality
Your privacy is respected. What you share in coaching will be treated with care and professionalism.
Coaching conversations are intended to remain confidential, except in situations involving legal requirements, risk of harm to self or others, or other circumstances where disclosure may be required by law.

Booking Policies
Please review all booking, cancellation, and session policies before your first appointment. By booking a session, you acknowledge and agree to the policies outlined in the coaching agreement and booking process.
Advance Booking: Sessions must be booked in advance.
Forms and Payment: Required forms and payment must be completed before your first session.
Policy Agreement: Booking confirms your agreement to the stated policies.
Cancellations: Cancellations made more than 24 hours before the scheduled start time are eligible for a refund. Cancellations made within 24 hours of the scheduled start time are non-refundable unless otherwise approved by Command Within.
